Waiver and Release of Liability I, the undersigned participant (or parent/legal guardian if under 18), acknowledge and agree to the following: Assumption of Risk: I understand that participating in a tennis lesson involves inherent risks including, but not limited to, slips, falls, heat-related illness, injuries from racquets or balls, and physical exertion. I voluntarily assume full responsibility for any risks, injuries, or damages that may occur as a result of participation in the Gripspin Tennis trial lesson. Release of Liability: In consideration of being permitted to participate in a trial tennis lesson offered by Gripspin Tennis, I hereby release and discharge Gripspin Tennis, its coaches, volunteers, staff, officers, and affiliates from any and all claims, demands, actions, or causes of action for injury, damage, or loss that may arise from participation in the lesson, whether caused by negligence or otherwise. Medical Treatment: In the event of an emergency, I authorize Gripspin Tennis and its representatives to secure medical treatment deemed necessary for myself or my child. I agree to be responsible for any costs related to such treatment. Media Release (optional): 1. I grant permission for Gripspin Tennis to take and use photographs or videos of myself/my child during the lesson for promotional or educational purposes. 2.
